QRAD’S Mission

QRAD’s mission is to help individuals ensure that their personal and end of life wishes are known and honored by providing education about the importance of advance directive completion. In addition, QRAD supports individuals in completing the documents and providing a way to universally store and retrieve their advance directives.

EDUCATE

Educating individuals, physicians, and other medical providers across the nation about the importance of having an advance directive and how to complete them.

ASSIST

Providing caring and compassionate assistance to individuals in determining their end of life wishes and correctly documenting their preferences in an advance directive.

COMMUNICATE

Ensuring that advance directives can be easily stored and retrieved via a program that facilitates communication between insurance companies, medical care providers, & all other parties.

Why QRAD was created…

QRAD got started during my doctoral program in social work at University of Southern California. While working as a Palliative Care social worker, I encountered individuals who did not have a completed advance directive or were unable to locate it. Individuals that I worked with expressed a lack of understanding about advance directives.

I have made it my mission to provide education and resources to individuals to achieve their end of life wishes. QRAD is poised to build trust and facilitate the sensitive conversations that are necessary to finalize an advance directive.
